Key changes for OT in the 2026 Medicare Part B proposed rule
On July 14, 2025, the Centers for Medicare & Medicaid Services (CMS) released their proposed Calendar Year (CY) 2026 Medicare Physician Fee Schedule (MPFS) Rule and Fact Sheet, which outlines policies for Medicare Part B coverage of OT services. Several of these proposals are a direct result of AOTA advocacy efforts, including:
- Addition of currently temporary OT CPT codes to the permanent Medicare telehealth code list
- New codes to capture shorter durations of Remote Therapeutic Monitoring
- Improved reimbursement rates due to a Congressional one-year increase
- Permanent tele supervision for OT practitioners (OTPs) in incident-to-physician practices.
Read below for a more detailed summary of these and other policy proposals that, if finalized, will impact the delivery of OT services under Medicare Part B. AOTA encourages members to learn more about these proposed changes and to comment directly to CMS on how these policy proposals will positively or negatively impact their OT practice.
