- Member rates apply to active members at the time of the Conference. If your membership is not in good standing at the time of the Conference, you will be responsible for paying the difference between the member and non-member rate.
- AOTA will not be offering group or speaker discounted rates.

Mailed registrations will be accepted through April 10th. After that date, please bring your registration form with payment to the Registration desk onsite in Salt Lake City.
When completing the Registration Form:
- Write legibly and complete both pages of the form.
- Complete a separate form for each person.
- Do not send more than one copy of your registration form, as this could result in duplicate registrations and duplicate credit card charges.
Fax the completed Registration Form to 770-238-0414. Due to the high volume of registrations we receive we do not confirm receipt of your fax. Please refer to the confirmation that your fax machine provides. You will receive a confirmation email when your registration has been processed.
If registrating online, by mail, or by fax is not possible, call AOTA at 800-729-2682, ext 2777, Monday–Friday, 9:00 am–5:00 pm ET, to register by phone. Please have all required information available when placing your call, including complete credit card information.
If you need to cancel your Conference registration, notify AOTA by email at firstname.lastname@example.org, or fax (770-238-0414). A 15% processing fee will be applied to all cancellation requests made through April 6, 2018. Refunds will not be issued after April 6, 2018 or for “no-shows.” Substitutions are accepted with a written notice. See “Registration Substitutions” below for instructions. Name Badges and Tickets
Name badges, event tickets and other conference materials will be picked up onsite. Attendees will check-in by scanning the barcode printed on the registration confirmation or by entering your last name. On-site registration hours are listed above.
Payment may be made by check or money order (payable to AOTA in U.S. funds) or by credit card. The following policies are in effect regarding payment:
An appropriate form of payment must accompany the Registration Form.
A fee of $25.00 will be assessed for any check returned by a bank for nonpayment.
If you expect your employer to pay your registration fee, give your employer ample notice for AOTA to receive payment before the registration deadline, or pre-pay your fees and receive reimbursement from your employer.
Confirmation/Receipt of Registration
When registering online, you will receive a confirmation email to the email address on record. Mail-in registrants will receive confirmation of registration upon processing of a completed registration form. Your confirmation letter will serve as your receipt of registration.
If you do not receive a confirmation letter within three days of registering, contact firstname.lastname@example.org, or fax a note to 770-238-0414.
If you find that you are unable to attend the Conference after you have already registered, you may designate another person to take your place, provided that the replacement meets the same registration fee criteria. If an additional fee structure applies to the substitute registrant, all additional fees must be paid at the time the substitution is made. Partial refunds will not be granted. Exchange of tickets for paid events will be considered, subject to certain restrictions.
The registrant must submit substitution changes in writing to AOTA or via fax, no later than April 10. After April 10 substitutions will be processed on-site.
